Mind & Body Reset, LLC – Financial & Treatment Policy By scheduling an appointment, you acknowledge and agree to the following: Mind & Body Reset, LLC is a private-pay, out-of-network healthcare practice. Neither Mind & Body Reset nor any of its providers participate with any insurance network. We do not accept Medicare or Medicaid. Patients are solely responsible for all charges regardless of insurance eligibility, verification, Assignment of Benefits (AOB), claim status, reimbursement, deductibles, copays, coinsurance, or denials. Insurance verification is provided only as a courtesy and is not a guarantee of payment. Insurance claims are submitted only upon patient request AS A COURTESY and only for eligible chiropractic services provided by Dr. Oneyke Berry, DC, and certain medically necessary massage therapy services when approved and provided under the direction of Dr. Oneyke Berry, DC. Submission of a claim does not guarantee reimbursement. If insurance is billed, you authorize Assignment of Benefits (AOB); however, you remain fully responsible for any unpaid balance. We accept Visa, Mastercard, American Express, Discover, cash, HSA/FSA cards, Apple Pay, Google Pay, Samsung Pay, Tap-to-Pay, Care Credit, HFC, Klarna, and Affirm (subject to approval). HSA/FSA eligibility is determined by your plan and IRS rules. Patients are responsible for verifying eligible expenses and providing an alternate payment method if an HSA/FSA transaction is declined. Ask about our discounted cash package options. Chiropractic adjustments, massage therapy, and integrative wellness services involve inherent risks, including temporary soreness, stiffness, muscle spasms, headaches, dizziness, fatigue, and, although rare, more serious complications. No guarantees are made regarding diagnosis, treatment outcomes, pain relief, recovery, or insurance reimbursement. NeuroSound Reset™, sound therapy, and other wellness services are wellness services and are not a substitute for medical, psychological, or emergency care. Cancellation Policy: We require 24 hours' notice to cancel or reschedule an appointment. Appointments canceled with less than 24 hours' notice and missed appointments ("No-Shows") may be charged up to the full appointment fee. Repeated late cancellations or no-shows may require prepayment before future appointments.
